The “reactive” Tripod Beta method facilitates the investigation and analysis of operational disturbances in order to prevent their recurrence. The strength of the Tripod Beta method lies in the identification of the “latent” or hidden failures in the organization that contributed to the operational disturbance, instead of focusing on just the immediate causes.
The attention is redirected from those directly involved in the incident, to the organizational failings. In most cases those directly involved are simply innocent bystanders to multiple factors coming together on that one moment, leading to the incident.
Based on the analysis, which is made visible in a Tripod Beta Tree, the active and latent failures are easily identified and shared with the organisation. In this way, the organisation is able to adequately manage its latent failures and decrease the likelihood of future incidents. |
